The FFMQ-SF is a 24-item questionnaire that measures five facets of mindfulness. Each subscale is made up of 5 items except for the Observe subscale, which has 4. Responses are rated on a 5-point Likert scale ranging from 1 (never or rarely true) to 5 (very often or always true).
