The SEMCD measures patients' self-efficacy to manage their chronic disease. It consists of six items on a 1 to 10 scale, with 1 anchored by ‘‘not at all confident’’ and 10 by ‘‘totally confident.’’ With paper versions of the questionnaire, participants circle a number, ‘‘For each of the following questions, please circle the number that corresponds with your confidence that you can do the tasks regularly at the present time.’’ With Internet (html) versions of the questionnaire, participants are asked to click on one of ten ‘‘radio buttons’’ associated with numbers 1 through 10.
