This scale measures chronic heart failure symptom severity. The scale is composed of12 items asking for 12 CHF symptoms: SOB with exertion, orthopnea, paroxysmal nocturnal dyspnea (PND), edema, weight gain, fatigue or weakness, cough, nausea, anorexia, dizziness, palpitation, and chest pain.
